Section 01
Getting Started โ€” Opening Your Tracker

The WellCharted Newborn to Age 5 Health Tracker is a single file that opens in any web browser on any device. No Canva account needed. No app to download. No login ever required.

Find Your Downloaded File

After purchasing from Gumroad you received a download link. Click it and save the file to your device. Check your Downloads folder. The file will end in .html

๐Ÿ’ก Tip: Move the file to your Desktop or a folder called "Child Health" so you can find it easily before appointments.

Open the File in Your Browser

Double-click the file and it opens automatically in your web browser โ€” Chrome, Safari, Edge, or Firefox all work perfectly. You will see the WellCharted tracker load with the navy blue header at the top.

๐Ÿ“ฑ On iPhone or iPad: tap the file โ†’ tap Share โ†’ tap "Open in Safari." On Android: tap the file in your Downloads app โ†’ opens in Chrome automatically.

Add to Your Home Screen for Easy Access

On your phone tap the Share icon then select "Add to Home Screen." The tracker will appear like an app icon โ€” one tap opens it instantly before every appointment.

๐Ÿ’ก This is the best tip in this guide. Parents who add it to their home screen actually use it before every visit!

Enter Your Child's Name First

Look for the child's name field near the top of the tracker and enter your child's full name. This will appear on printed pages so providers can identify the record immediately.

โœ… You're Ready!

Once you see the navy header with the tab names across the top โ€” your tracker is open and ready. Start with the Well Visits tab to log your child's first appointment.

Tracking Vaccines Step by Step

The vaccine tab is one of the most important sections in this tracker โ€” especially for foster families, adopted children, or anyone managing incomplete records. Here is how to use it.

Log Each Vaccine Right After the Visit

As soon as your child receives a vaccine click + Add Vaccine and enter the vaccine name, date, dose number, and the facility where it was given.

Transfer Your Existing Paper Records

If your child already has a paper vaccine card โ€” sit down and transfer all the past vaccines into the tracker. This creates a digital backup that you can print or share at any time.

๐Ÿ“Œ This is especially important for foster and adoptive families โ€” even partial records are better than none. Enter what you have.

Check the Reference Schedule

The tracker includes the CDC childhood immunization schedule built in. Use it to check which vaccines are coming up at your child's next visit so you can ask the provider in advance.

Print Your Vaccine Record

Go to the Vaccines tab โ†’ click ๐Ÿ–จ Print / Save PDF. This prints a clean vaccine history you can bring to a new school, daycare, or new provider who needs records.

A Special Note for Foster & Adoptive Families

This tracker was designed with your unique situation in mind. Managing incomplete records, multiple placements, and new providers is challenging โ€” this tool helps you build a complete picture one piece at a time.

You Don't Need Complete Records to Start

Many foster and adoptive families receive children with little to no health history. That's okay. Start logging from today and build forward. Here's what to do:

Request Records Right Away

Contact the placing agency, previous foster family, or birth parent's known providers and request all available medical records. Even partial records help fill in the vaccine and health history tabs.

Schedule a Complete Well Visit

Request a comprehensive well-child exam as soon as possible after placement. Bring the tracker โ€” even if it's mostly empty โ€” and ask the provider to help document what they find.

๐Ÿ’ก Tell the provider "I am the foster parent and I have limited health history. Can we document everything today?" Most providers will go the extra mile.

Use the Notes Field Generously

In any tab that has a notes field โ€” use it. Document where information came from, what's uncertain, and any observations you've made since placement. This context is invaluable for future providers.

The tracker's print feature means you can produce a clean, organized health summary for court hearings, agency reviews, or reunification planning. Print the Well Visits, Vaccines, and Medications tabs together as a complete health portfolio.

How to Print Any Tab

Every tab is print-ready. Print before appointments, for school records, daycare enrollment, or to share with specialists.

Go to the Tab You Want to Print

Click the tab name at the top โ€” Well Visits, Vaccines, Medications, etc. Make sure you can see the information on screen.

Click Print or Press Ctrl+P

Click the ๐Ÿ–จ Print / Save PDF button in the top right โ€” or press Ctrl+P (Windows) or Cmd+P (Mac). Only the tab you are currently viewing will print.

Save as PDF Instead of Printing

Choose Destination: Save as PDF in the print window to save a digital copy. Perfect for emailing to a new school, specialist, or agency without printing on paper.

Frequently Asked Questions
Do I need a Canva account to use this?
No โ€” absolutely not. This tracker has nothing to do with Canva. It is an HTML file that opens directly in your web browser. No account, no login, no subscription needed ever.
Does it work on my phone?
Yes. Open the file in your phone's browser. Tap Share โ†’ Add to Home Screen and it appears like an app. Works on iPhone, Android, tablet, and computer.
Do I need the internet to use it?
No. Once saved on your device it works completely offline. Perfect for doctor's offices with poor wifi.
Is my child's information private?
Yes โ€” completely. Your data is saved only on your device and is never sent to any server, website, or cloud. Only you can see it.
Can I use one tracker for multiple children?
Each child should have their own separate copy of the file to keep records separate. You can purchase additional copies and name each file with the child's name.
What if I don't have the child's full vaccine history?
Enter whatever you have and note any gaps in the notes field. Your child's pediatrician can help catch up on missing vaccines and fill in gaps at the next well visit.
Will my data be lost if I accidentally close the browser?
No โ€” your data saves automatically as you add entries. Closing the browser is fine. Just reopen the file and everything will be there. Avoid clearing your browser's cookies and site data as that can erase saved information.
Is this a substitute for medical advice?
No. This tracker is a personal organization tool to help you communicate with your child's healthcare provider. Always consult your child's pediatrician for medical decisions.
