Section 01
Getting Started โ€” Opening Your Tracker

The WellCharted Health Confidence Tracker is a single HTML file โ€” meaning it lives right on your device and opens in any web browser. No Canva account needed. No app to download. No login ever required.

Find Your Downloaded File

After purchasing from Gumroad you received a download link. Click it and save the file to your device. It will be named something like index.html or health-tracker.html. Check your Downloads folder.

๐Ÿ’ก Tip: Move the file somewhere easy to find โ€” like your Desktop or a folder called "Health." You'll want to open it again later.

Open the File in Your Browser

Double-click the file. It will open automatically in your web browser โ€” Chrome, Safari, Edge, or Firefox all work perfectly. You'll see the WellCharted Health Confidence Tracker load with the navy blue header at the top.

๐Ÿ“ฑ On iPhone or iPad: tap the file โ†’ tap Share โ†’ tap "Open in Safari." On Android: tap the file in your Downloads app โ†’ it opens in Chrome automatically.

Bookmark It for Easy Access

Once the tracker is open in your browser press Ctrl+D (Windows) or Cmd+D (Mac) to bookmark it. On your phone tap the share icon and select "Add to Home Screen" โ€” it will appear like an app icon on your home screen.

๐Ÿ’ก Tip: Adding to your home screen is the easiest way to open it quickly before appointments.

Dismiss the Disclaimer Banner

At the very top you will see a yellow/gold banner with an important disclaimer. Read it โ€” then click the โœ• on the right side to dismiss it. It will appear each time you open the tracker as a reminder.

โœ… You're Ready!

Once you see the navy header with all the tab names across the top โ€” your tracker is open and ready to use. No account needed, nothing to sign up for.

Your 15 Tabs โ€” What Each One Does

At the top of the tracker you will see tabs โ€” click any tab to go to that section. Here is a quick guide to what each one is for.

PCP Visits
Track your quarterly routine check-ups and any sick or urgent visits. Click โœ๏ธ Edit to fill in appointment details for each quarter.

Top 3 Questions
Write your questions before each appointment. Enter your name and date at the top then print to bring with you. Mark each question reviewed after your visit.

A running log of past conditions, treatments, surgeries, and providers. Great for new provider visits.

Log any unscheduled visits, ER trips, or walk-in clinic visits with the reason, facility, and outcome.

If you're just getting started don't try to fill everything in at once. Begin with Medications, Health History, and Top 3 Questions โ€” those three will make the biggest difference at your very next appointment.

Section 03
How to Add Information

Every tab works slightly differently but they all follow the same simple pattern. Here's how to add, edit, and delete entries throughout the tracker.

Using the + Add Button

Most tabs have a blue + Add button at the bottom. Click it and a window pops up with fields to fill in. Enter your information and click Save. Your entry appears in the list immediately.

๐Ÿ’ก Examples: "+ Add Medication", "+ Add Referral", "+ Add Screening" โ€” all work the same way.

Using the โœ๏ธ Edit Button (PCP Visits)

On the PCP Visits tab each quarter has an โœ๏ธ Edit button. Click it to open a window where you can enter your appointment date, provider name, notes, and status for that quarter.

Changing the Status

Many tabs have a Status dropdown โ€” a small clickable button that says things like "Scheduled", "Pending", or "Completed." Click it and choose the right status from the list. It saves automatically.

Deleting an Entry

Every entry has a small ๐Ÿ—‘ trash icon on the right side. Click it to remove that entry. It deletes immediately โ€” so double check before clicking!

Inline Editing (Medications Tab)

On the Medications tab the Notes column can be edited by clicking directly on the field and typing. Your changes save automatically when you click away.

Section 04
Using the Top 3 Questions Tab

This is one of the most powerful features in the tracker. Use it before every appointment to prepare your questions โ€” then print and bring it with you.

Enter Your Name and Appointment Date

At the top of the Questions tab you will see a bar with two fields โ€” Patient Name and Appointment Date. Fill these in. They will print at the top of your questions sheet so your provider can see your name.

Type Your Questions

There are 3 question cards numbered 1, 2, and 3. Click in each large text box and type your question. Be as specific as you can โ€” for example: "My knee has been swollen for 2 weeks โ€” should I have an X-ray?"

๐Ÿ’ก Tip: Write your most important question as #1. If you run out of appointment time at least your biggest concern was addressed first.

Save Your Questions

Click the blue ๐Ÿ’พ Save Questions button. Your questions are saved to this browser and will still be there next time you open the tracker.

Print and Bring to Your Appointment

Click ๐Ÿ–จ Print for Appointment. Your questions sheet will print with your name, appointment date, each question, and a space for the provider's plan. Hand it directly to your provider or nurse.

Fill in the Provider's Plan and Mark Reviewed

After your appointment type what your provider said in the Provider's Plan / Response field under each question. Then click the green โœ“ Mark Reviewed with PCP button โ€” the question gets a green "Reviewed" badge.

๐Ÿ’ก This creates a record of what was discussed and agreed to at each visit. Very helpful for follow-up.
Section 05
How to Print Any Tab

Every tab in the tracker is print-ready. You can print any section to bring to an appointment, share with a specialist, or keep in a physical binder.

Go to the Tab You Want to Print

Click the tab at the top of the tracker โ€” for example Blood Pressure, Medications, or Lab Results. Make sure you can see the information you want to print on screen.

Use the Print Button or Ctrl+P

Either click the ๐Ÿ–จ Print / Save PDF button in the top right of the header โ€” or press Ctrl+P on Windows or Cmd+P on Mac. Both open the same print window.

๐Ÿ“Œ Only the tab you are currently viewing will print โ€” not all 15 tabs at once.

Choose Your Print Settings

In the print window you can choose your printer or select "Save as PDF" to save a digital copy. For best results select Portrait orientation and make sure Background graphics is turned on so the colors print correctly.

Save as PDF Instead of Printing

To save a digital copy instead of printing on paper โ€” in the print window choose Destination: Save as PDF then click Save. This creates a PDF file of just that tab that you can email to a specialist or save for your records.

Saving Your Information

Your information saves automatically as you type โ€” you never need to click a special save button for most things. Here is what you need to know about how your data is stored.

Right in your browser on your own device โ€” called "local storage"
Is it private?
100% โ€” your data never goes to any server or cloud. Only you can see it.
Does it save automatically?
Yes โ€” most fields save the moment you add or change them
What if I clear my browser?
Clearing browser history/cache may erase your data โ€” avoid this
Can I use it on multiple devices?
Each device saves separately โ€” your phone and computer won't sync automatically
How to back up your data
Print key tabs to PDF regularly and save those files as your backup
โš ๏ธ Important โ€” Don't Clear Your Browser Data

If you use your browser's "Clear browsing data" or "Clear history" feature and select "Cookies and site data" โ€” this will erase your tracker information. To be safe regularly print your key tabs to PDF as a backup copy.

๐Ÿ’ก Best Practice for Keeping Your Data Safe

Once a month โ€” or before any major appointment โ€” print your Medications, Health History, and Lab Results tabs to PDF and save them in a folder on your computer or in your email. This gives you a permanent backup that doesn't depend on the browser.

Frequently Asked Questions

Answers to the most common questions about using your tracker.

Do I need a Canva account to use this?
No โ€” absolutely not. This tracker has nothing to do with Canva. It is a simple HTML file that opens directly in your web browser. No account, no login, no subscription of any kind is needed.
Do I need to be connected to the internet?
No. Once the file is saved on your device it works completely offline. You only need internet to load the fonts the first time you open it โ€” after that it works anywhere, even without wifi.
What browser should I use?
Any modern browser works โ€” Chrome, Safari, Firefox, or Edge. Chrome and Safari tend to give the best print results. On iPhone use Safari. On Android use Chrome.
Can I use it on my phone?
Yes. Open the file in your phone's browser. For the easiest access tap Share โ†’ Add to Home Screen and it will appear like an app on your home screen. The tracker is designed to work on phones, tablets, and computers.
I accidentally deleted something โ€” can I get it back?
Unfortunately no โ€” deleted entries cannot be recovered. This is why printing key tabs to PDF regularly is a good habit. Going forward entries delete immediately when you tap the ๐Ÿ—‘ icon.
Can my doctor or family member see my information?
Only if they have access to your device and browser. Your data is stored only on your device and is completely private. Nothing is transmitted to any website or server.
Can I use this for a family member?
Yes โ€” but each person should use a separate copy of the file on their own device so their data stays separate. You can share the download link with family members who purchased their own copy.
The tabs at the top โ€” I can't see all of them. What do I do?
Use the โ€น โ€บ arrow buttons on either side of the tab bar to scroll left and right. On a phone you can also swipe the tab bar sideways to see more tabs.
Is this a substitute for medical advice?
No. This tracker is a personal organization tool to help you communicate better with your healthcare provider. It does not provide medical advice, diagnoses, or treatment recommendations. Always consult your primary care provider for medical decisions.
